Mount Sinai Partners with Livongo Health on Digital Diabetes Management Program

New York City-based Mount Sinai Health System has teamed up with consumer digital health company Livongo Health, based in Mountain View, Calif., to a launch a digital health diabetes management program.

New York City-based Mount Sinai Health System has teamed up with consumer digital health company Livongo Health, based in Mountain View, Calif., to a launch a digital health diabetes management program.

The diabetes management program combines the Livongo for Diabetes solution with the clinical capabilities of the Mount Sinai Clinical Diabetes Institute and the Mount Sinai’s Diabetes and Cardiovascular Alliance to offer a health management solution to patients with diabetes. The program, which had already been available to Mount Sinai employees, will be expanded to broader populations across the New York market through Mount Sinai’s network of physicians and affiliated providers, according to a Mount Sinai press release.

The program is built on a smart data analytics platform that captures information from a variety of sources, including Livongo’s connected glucose meters, personal devices, electronic health records and medication records.  Relevant and timely insights and alerts are then provided in real-time to the person with diabetes, their network of family and friends, and their Mount Sinai care team.

"With several hundred thousand individuals covered under our value-based care arrangements, and that number growing rapidly, we are shifting our care models accordingly, and Livongo will play a key role in supporting our efforts to effectively manage diabetes—a chronic health condition that affects more than 10 percent of New Yorkers,” Niyum Gandhi, executive vice president and chief population health officer, Mount Sinai Health System, said in a statement. “Mount Sinai remains committed to transforming healthcare in New York, and we are proud to collaborate with innovative companies such as Livongo to help us meet the goals of our health system and the community.”
