Humana, a nationwide provider of Medicare benefits, is rolling out a self-care program for Medicare Advantage diabetes patients, in partnership with patient engagement platform provider dLife Healthcare Solutions. The platform, called The My Diabetes Path (MDP), has already launched in pilot for more than 18,000 member participants, providing multi-channel, multimedia interventions using dLife’s content, tools, and online community for diabetes self-management.
Overall, early results from the pilot, Humana says, showed that there was improvement in various diabetes testing. Members had a seven percent improvement in LDL screening, nine percent improvement in blood sugar screening, seven percent improvement in eye exams, and six percent improvement in kidney disease monitoring.
Kathryn Creech, vice president of quality and consumer engagement at Humana, said in a statement: “My Diabetes Path is an opportunity Humana is now offering to Medicare Advantage members with diabetes — to have the tools and resources to manage their diabetes better and live longer, healthier lives.”
The platform, Humana says, is aimed at improving awareness, engagement, and education on diabetes self-management and control. It looks to target improvements in compliance with recommended screenings, A1C and LDL control, member knowledge about diabetes self-care, and adoption of positive self-care practices that will help members improve their health and well-being.
