After conducting a detailed review of the incident, which affected patients between May 2010 and October 2013, Phoebe determined that the computer might have contained patient information, including patient names, addresses, dates of birth, dates of service, physician’s names, and diagnosis information.

Phoebe contacted the Albany Police Department and hired a computer forensics company to identify the information contained on the computer. To date, the computer has not been located, the hospital confirmed in a statement.
