Gundersen Lutheran Deploys Patient Outreach System

La Crosse, Wyo.-based Gundersen Lutheran Health System has launched Dallas-based Phytel’s Proactive Patient Outreach solution.

According to the company, the solution automates the follow-up process for a practice’s patient population, specifically patients with chronic conditions and those in need of preventive care services. Patients in the registry are notified through automated outreach messaging when they are due for recommended care.

The solution integrates with existing technology, including practice management and EMR systems, and incorporates prescription data, lab data and other clinical data, according to the company.
