MED3OOO Inc. recently purchased the Trizetto Facets e2 enterprise software and Clinical CareAdvance care management module to enhance population health management initiatives with its independent practice association (IPA) and health plan clients.
MED3OOO, a national healthcare management and technology company based in Pittsburg, will initially begin deploying these solutions with Health First Network in Pensacola, Fla.
The population health management initiatives, which include disease management, case management, and predictive modeling, will help the IPA to identify patients at risk for advanced disease and multiple morbid conditions, MED3000 contends.
