The New York City Health and Hospitals Corporation (HHC), an integrated delivery system serving 1.3 million New Yorkers, launched a Web site designed to help diabetic patients become more active participants in their care and access tips and resources to manage their blood sugar.

According to the organization, the HHC Diabetes Wellness Center features advice from HHC doctors, nurses and nutritionists, patient success stories, healthy eating and exercise tips, access to free blood monitors, and a list of support groups and diabetes wellness classes available in the system’s 11 public hospitals.

The site will be promoted at HHC hospitals and clinics, and through direct mail to more than 58,000 diabetic patients that are monitored through HHC’s Diabetes Patient Registry, an electronic disease-tracking program designed to improve physicians’ ability to monitor and more effectively manage diabetes among patients.

The New York City Health and Hospitals Corporation is the largest municipal healthcare organization in the country. It provides medical, mental health and substance abuse services through its 11 acute care hospitals, four skilled nursing facilities, six large diagnostic and treatment centers and more than 80 community based clinics.
