According to findings from a pilot published in the journal Health Affairs, Geisinger Health System (Danville, Pa.) reduced hospital admissions by 20 percent and saved seven percent in total medical costs by providing a patient-centered medical home model of care.
The model, which included around-the-clock access to primary and specialty care and physician and patient access to EHRs over the course of a year, is part of an effort by the organization to redesign its care delivery infrastructure and to create incentives aligned to enhanced healthcare value, it says.
