Health Network Taps Phytel for Patient Follow-up

Holy Family Memorial (Manitowoc, Wisc.) has selected Proactive Patient Outreach from Dallas-based Phytel to automate its patient follow-up process.

Holy Family Memorial (Manitowoc, Wisc.) has selected Proactive Patient Outreach from Dallas-based Phytel to automate its patient follow-up process. The 65-provider, multi-specialty group plans to implement the technology by mid-March.

According to the company, the solution will automate the follow-up process at Holy Family by using evidence-based disease management and preventive care protocols to identify patients who are overdue for service and notifying them through automated outreach messaging. Proactive Patient Outreach will also enable the health network to track patient response and monitor compliance, says the company.
