Foster City, Calif.-based Health Access Solutions, offering a Web-based multidisciplinary care coordination platform, announced a partnership with the Sacramento-based California Association of Physicians Group (CAPG). CAPG, representing more than 150 physician groups that provide care to nearly 15 million patients throughout the state, has endorsed the Health Access Solutions multidisciplinary care coordination platform—which combines the case and utilization management components of Access Express with the clinical functionality of Excelicare—as an effective solution to facilitate accountable care.

Recent healthcare reform legislation, including the Patient Protection and Affordable Care Act (PPACA), highlights the importance of and critical need for a patient-centric, multidisciplinary delivery system to coordinate care while balancing quality, utilization of services and cost. Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) models are outlined within PPACA as effective methods to achieve this degree of collaboration, as is the need to develop effective health information technologies to support these forward-looking strategies.

CAPG, as co-producer of the National ACO Congress being held October 25-27 in Los Angeles, is committed to elevating healthcare in California—and the United States—with the ACO-style care environment.
