La Crosse, Wis.-based Gundersen Lutheran Health System launched a patient outreach solution from of Dallas, Texas-based Phytel.

According to the company, the solution automates the follow up process for a practice’s patient population, specifically patients with chronic conditions and those in need of preventive care services.


The solution integrates with existing technology, including practice management and electronic medical record systems, and incorporates prescription data, lab data, and other clinical data.
