Chicago-based Allscripts has announced that Vanderbilt University Hospital of Nashville, Tenn., has selected Allscripts Care Management’s discharge planning solution at its facility. The Web-based software-as-a-service will connect Vanderbilt online with more than 90,000 post-acute care providers nationwide, automating the entire process of discharge from end to end.

During a patient’s hospital stay, Allscripts Care Management software will provide Vanderbilt’s case managers a with a concise view of the patient’s post-acute care in order to design the best discharge plan, streamlining a process that can be inefficient, costly, and time consuming. Added data management capabilities of the system include staff productivity reports and the ability to track the number of referrals to post-acute facilities.

More than 160,000 physicians, 800 hospitals and 10,000 post-acute and homecare organizations utilize Allscripts.
