Atlanta-based RelayHealth has launched IntegrateRx - Medication History, an electronic medication reconciliation solution.

According to RelayHealth, the solution is designed to eliminate blind encounters in hospital emergency rooms by providing a record of a patient's medications based on information extracted from insurance claims and/or prescription records.

New York-based Duane Reade is among those pharmacies that have agreed to make their patients' medication history records available to authorized staff at hospital emergency rooms.
