Today, Cerner Corp. and Qualcomm Life, Inc. announced a new agreement that will extend Cerner's medical device connectivity capabilities to better serve chronically ill patients. With the use of Qualcomm Life's FDA-listed 2net™ Platform and Hub, Cerner will now be able to seamlessly capture data from medical devices and sensors at patients' homes, including weight, blood pressure, pulse oximeters and other measures, and transmit via the CareAware® platform to the Millennium® electronic health record (EHR). This partnership will allow care providers to better monitor patients in real-time and prevent unnecessary trips to the hospital.

Agnesian Healthcare will be one of the first clients to implement this new initiative, moving towards a realization of widespread population health management.
