California’s Manifest MedEx to Launch Statewide Social HIE

Collaboration with NinePatch to integrate clinical, behavioral health, administrative, and social data within Manifest MedEx’s network

California health information network Manifest MedEx (MX) will begin offering a statewide social health information exchange (SHIE) platform to California county health agencies in 2026. 

MX is a designated qualified health information organization (QHIO) under California’s Data Exchange Framework (DxF). Providers and health organizations, including community-based organizations (CBOs), that participate in the MX network will be able to share an individual’s health history, social needs, and social services information, as permitted. MX said the platform’s integrated tools will combine this information into one unified view, helping healthcare providers, social service providers, and Medi-Cal plans deliver Enhanced Care Management and Community Supports services under CalAIM more effectively. (CalAIM is the state’s multi-year Medicaid transformation effort.)

One large county in California’s Central Valley will be the first to implement MX’s SHIE, advancing the region’s community data exchange strategy to support public health and CalAIM goals. The county plans to deploy the platform across county departments and contracted CBOs, enabling them to securely track, report, and share social data while maintaining strict permissions that limit CBO access to sensitive health data. The SHIE platform supports appropriate role-based access to enable coordination across multiple county departments and social services organizations.

MX is working with Colorado-based vendor NinePatch  on the SHIE platform. Launched out of community work in Colorado, NinePatch said it focuses on building relationships across health and social care channels to promote whole person care. Through client needs identification, closed loop referrals, care coordination, programs and services, case management and documentation, robust reporting, billing, and outcome measurements, NinePatch said it provides end-to-end capabilities that empower communities to transform the lives of the people they serve.

The SHIE platform will also be made available to MX’s network of more than 140 hospitals, 19 health plans, 12 county health agencies, and 2,700 providers across the state.

In addition to its partnership with NinePatch for its SHIE, MX is also collaborating with Findhelp, a social care referral platform, to support seamless, secure, and appropriate access to SDOH data for participants in the MX network.
