Trinity Health, Heritage Looking to Take Population Health to National Level
Trinity Health, the Livonia, Mich.-based large Catholic health system, is forming a joint population health partnership with the Heritage Provider Network, an integrated physician group based in Marina del Rey, Calif.
The two provider organizations will take risk-based coordinated, accountable care to a national level, they said in the announcement. They plan on working with local insurers to help set up risk-based contracts for primary care, care management, hospitalist, post-acute care and high-risk clinics. According to Modern Healthcare, the joint venture has already begun conversations with various insurers. They plan on starting in a few markets and eventually expanding beyond that.
“We are excited to offer Heritage’s population health capabilities to the broader population served by Trinity Health's expansive national hospital system," Richard Merkin, M.D., president and CEO of Heritage Provider Network, said in a statement. "Our proven methodology combined with Trinity Health's broad network will offer the opportunity to deliver affordable health and wellness to more people."
Trinity and Heritage are organizations that have already made significant movement in population health management, with both having accountable care organizations (ACOs) that are part of Centers for Medicare & Medicaid Services (CMS) programs. Heritage is a Pioneer ACO and a group within Trinity was just announced as one of the 89 new Medicare Shared Savings ACOs.
