Serviam, Curana Launch Value-Based Care Alliance of Florida

Effort seeks to bring senior living operators and healthcare providers together to implement value-based care in senior housing settings

Serviam Care Network and Curana Health have launched the Value-Based Care Alliance of Florida, an initiative designed to transform senior care across the state. 

The effort seeks to bring senior living operators and healthcare providers together to implement value-based care in senior housing settings (to improve health outcomes for senior living residents while reducing operator costs).

Kansas City-based Serviam was created to serve a coalition of senior living providers with the purpose of transforming how America cares for seniors. The company describes itself as a company building an ecosystem of technology and services that allows senior living communities to thrive within a value-based care system The network currently includes 41 operators across 49 states and more than 90,000 lives. 

Austin, Texas-based Curana was formed in 2022 when three organizations that provide healthcare services to senior living communities – Elite Patient Care, Provider Health Services, and AllyAlign Health – joined forces to drive the growth of value-based healthcare models in senior living communities.

The combined Curana Health organization serves approximately 175,000 patients annually, spanning 34 states and more than 1,600 senior living sites.

The partner companies launched a similar alliance in Virginia in 2023. At the core of the Alliance is Serviam's HigherPath model of care, an approach that is designed to help seniors maintain the highest possible level of wellness, instead of waiting until they get sick to provide care, which is often more complex and expensive, while providing hands-on support at the local level. Essential to unlocking the HigherPath is Curana's proactive, team-based approach to highly coordinated patient care that is proven to enhance the care experience for senior living residents, improve clinical outcomes, and reduce unnecessary and often harmful health care spending.

At the heart of the initiative is Curana Health's on-site primary care services, which include physician-led care teams supported by Curana's Medicare Accountable Care Organizations and Medicare Advantage Special Needs Plans (designed specifically for residents of senior living communities).Through these integrated services and Curana's value-based care programs, the Alliance seeks to equip operators with the tools and support needed to succeed in value-based care—without requiring upfront capital, extensive relationships, or a high volume of patients.
