Memorial Hermann, Monogram Partner on In-Home Care
Memorial Hermann Health System has formed a joint venture with Monogram Health, which provides multispecialty in-home care. Together, they will provide care for commercial and Medicare Advantage members of Memorial Hermann Health Plan, the insurance division of the Houston-based health system.
The goal of the partnership is to deliver holistic, patient-centered care that addresses the complex needs of individuals living with chronic kidney disease and its causes, including diabetes, high blood pressure and metabolic disorders.
Memorial Hermann owns and operates 14 hospitals and has joint ventures with three other hospital facilities, including Memorial Hermann Surgical Hospital First Colony, Memorial Hermann Surgical Hospital Kingwood and Memorial Hermann Rehabilitation Hospital-Katy.
"This milestone marks an exciting first step for our joint venture with Memorial Hermann Health System," said Mike Uchrin, CEO and co-founder of Nashville-based Monogram Health, in a statement. "We're creating a seamless care model that holistically and collaboratively addresses complex health needs and improves outcomes for Memorial Hermann patients living with chronic diseases."
The goal is to expand the program to additional populations and health plans.

Monogram is privately held by Frist Cressey Ventures, Norwest Venture Partners, TPG Capital, as well as other investors and now covers 200,000 lives in 36 states.
