Summary
Decades ago, consumers transitioned from paper maps to digital GPS for navigating travel. Although healthcare journeys — like pre/post-surgery, oncology and chronic care — are far more complex, many patients still rely on paper instructions and verbal guidance that are easily lost or forgotten. Meanwhile, care teams struggle to support patients outside the clinic or hospital, often missing the chance to catch patients who fall off track. In this webinar, discover how leading health systems are utilizing digital care journey platforms, fully integrated with EHRs, patient portals, and digital front doors, to provide patients with a digital co-pilot throughout various surgical procedures and health conditions. Hear from change management and clinical experts about the nuts and bolts of implementing this digital health approach, achieving high patient adoption while reducing length of stay, readmissions, mortality, and costs, all while increasing profitability and patient satisfaction.
