Minneapolis-based PointClickCare and Chicago-based Allscripts have completed the integration of the Inquiry Referral Management (IRM) module, a new system linking data between hospitals and long-term care facilities.

With the electronic link, EHRs can be utilized to decrease the amount of time a patient has to wait to be transferred from a hospital setting to a long-term care facility and cut down on errors, according to the companies.

Together, PointClickCare and Allscipts say they have the capability to link 40,000 physicians with 6,000 post-acute agencies (including home care), 500 major hospitals and 3,000 long-term care facilities.
