Blue Cross BlueShield of Tennessee is implementing a population health management platform from Care Team Connect to improve the overall quality of care at its patient-centered medical home (PCMH) program while also reducing costs for those members. The platform will also allow BlueCross to streamline various pay for performance programs.
Care Team Connect (CTC), which was recently deemed an Up-and-Comer by Healthcare Informatics, says it has a platform that allows providers and payers to track and measure performance against a core set of measures. At Michigan Pioneer accountable care organization (ACO), care managers use the platform to track home care and sub-acute care to create a community-based electronic record.
“Care Team Connect’s end-to-end care management solution truly stands apart in the population health management space with its ability to drive action by leveraging both payer and provider data. Additionally, CTC’s platform enables us to share data and collaborate with our providers in real time,” Christy Vitulli, vice president of provider networks at BlueCross, said in a statement. “CTC’s workflow management and coordinated care capabilities have already demonstrated success with providers in Tennessee and beyond, which further validated our selection.”
