The Advisory Board Company has acquired Care Team Connect, an Evanston, Ill.-based vendor with a web-based platform to devise treatment plans for patients as they navigate between hospitals, doctors and home.

Care Team Connect will join Advisory Board’s hospital-physician alignment performance technology, Crimson, which brings analytics to 500,000 physicians. The result is a new offering called Crimson Care Management, which will aim to help providers deliver value-based care by allowing collaborative care teams to apply population-level risk stratification directly in their day-to-day patient care and deliver best practice interventions, officials say.

Care Team Connect founder Ben Albert said he was inspired to start the company to address the lack of coordinated care he witnessed after his grandfather suffered a stroke. Care Team Connect was named as an up-and-coming vendor in this year’s Healthcare Informatics 100 list.
