Humana and Northwest Physicians Network Partner on Accountable Care

Northwest Physicians Network (NPN), a Washington state-based physicians association, and Humana have formed an Accountable Care agreement that will provide Humana Medicare Advantage members access to NPN’s more than 500 providers throughout the Puget Sound region.

Northwest Physicians Network (NPN), a Washington state-based physicians association, and Humana have formed an Accountable Care agreement that will provide Humana Medicare Advantage members access to NPN’s more than 500 providers throughout the Puget Sound region.

NPN is the largest multi-specialty independent practice association in the Northwest and includes 200 ancillary facilities.

The goal of the Accountable Care agreement, according to both companies, is to improve care for Washington state seniors.

The agreement with NPN is part of Humana’s value-based model that promotes evidence-based care with a focus on physician support for successful population health management. The value-based model is designed to improve cost, quality and the patient experience.

“Humana is excited to work with NPN to continue delivering value-based health care to our Medicare Advantage members in the Puget Sound region,” Catherine Field, Intermountain vice president for senior products at Humana said in a statement. “This model is designed to foster high-quality healthcare, lower costs and provide a more patient-centric healthcare system.”

The new Humana-NPN accountable care agreement includes value-based incentives tied to performance improvement and supports successful population health management. The arrangement also aligns NPN and Humana’s complementary population health capabilities, including predictive analytics, chronic care, disease management and wellness programs.
