Lumeris collaborates with Mount Sinai Health System to advance population health management initiatives

Lumeris announced a multi-year collaboration with Mount Sinai Health System to provide value-based care managed services. Under the terms of the agreement, Lumeris will provide operational support enabled by its industry-leading technology, to further Mount Sinai’s population health management objectives.

Niyum Gandhi, executive vice president and chief population health officer for Mount Sinai Health System, shared, “As we rapidly embrace value-based care as a system, continuously improving our efficiency and quality to improve the overall health of our community is a critical priority.”

Using Lumeris’ business model to provide better outcomes for consumers and reduce costs, Mount Sinai anticipates doubling the number of lives covered under value-based arrangements.
