The healthcare industry’s rocky journey into data-driven value-based care has proven time and again that healthcare data fragmentation is the enemy of true value. Without being able to access, analyze, and act upon meaningful data, healthcare providers and patients find themselves stuck in a cycle of waste, confusion, frustration, and poor outcomes.

That’s where Holon comes in.

Holon is a unique new class of system that is purpose-built to enable users to receive relevant, impactful hard-to-access care delivery knowledge they need in a way that is simple for them to use. We are fully dedicated to creating a precise, intelligent healthcare knowledge supply chain that overcomes fragmentation and unifies the entire network of partners involved in each individual’s care to enable maximum outcomes for everyone.

Our flagship innovation is CollaborNet, a robust suite of connectivity and applications that employs patented, workflow sensor technology to gather and deliver curated, precise patient information when and where it’s needed.

By surfacing contextualized, personalized, multisource patient insights directly to the user, CollaborNet makes sure that providers don’t just have access to necessary data – they actually have the ability to use it.

CollaborNet has five key components that create an innovative healthcare knowledge supply chain, starting with workflow sensors that detect users, their tasks, and their subjects to conduct dynamically configured, intelligent search of available data from predefined sources.

The central brain of our system, called Hive, manages the sensors and access to the authorized data sources. We utilize HL7 FHIR to define our data resources so customers can use what they have today while preparing for ongoing innovations in interoperability.

Access to data is only a small part of the solution. Insights must be surfaced to the end-user in an intuitive, workflow-friendly manner. Our Ribbon technology is a configurable display of information that connectsusers to the knowledge they need as we gather it for them.

These tools are paired with Dynamic Conversational Messaging to conduct interactive transactions, such as referrals or authorizations, and Dynamic Links which provide the users with additional authorized and contextual links to external systems for supplemental information or functions as they need them.

The team behind these innovations has deep and varied experience in the healthcare technology world. CEO Jon Zimmerman has spent more than 35 years devising solutions to some of healthcare’s greatest challenges, working with companies such as IBM, Siemens, CareFusion, Allscripts, Availity, and GE to better serve providers and patients.

In addition, Holon has a stellar group of proven leaders with diverse and impactful backgrounds in healthcare and technology.

Key members of the Holon innovation team include Robert Connely, who has close to 20 years of healthcare strategy and business development experience. Robert has been instrumental in securing key clients and business partnerships as the subject matter expert for Holon’s channel, OEM, and VAR partners. Rob has authored seven Holon patents and has testified in USPTO office actions, resulting in four patent grants to date.

Nate King, Holon’s Chief Product & Technology Officer, has also been invaluable for Holon’s success. With more than two decades of experience in enterprise class system architecture design and numerous highly successful product launches on his record, Nate’s leadership and creativity have been essential for Holon’s growth and development.

Healthcare providers, health systems, and payers have all seen real-world results from the work of the Holon team. For example, G-Health Enterprises, a group of healthcare affiliates in Western New York, is leveraging Holon’s referrals and insights solutions to deliver comprehensive, proactive care to traditionally underserved populations.

With Holon’s solutions, G-Health has enabled real-time referral management between clinical providers and community based organizations (CBOs), which don’t typically have EHRs. Closing the referral loop and ensuring patients access services that address the social determinants of health allows G-Health to improve health equity and foster better outcomes for vulnerable populations.

For G-Health and numerous other partners just like them, Holon is improving communication and creating tangible value for patients, providers, and payers. Breaking the cycle of data fragmentation, financial waste, and cognitive overload for providers is crucial for achieving the goals of valuebased care.

CollaborNet’s ability to deliver precise, actionable, timely insights directly to providers at the point of care is central to success in the modern healthcare environment, and Holon is extremely proud to be equipping providers with the knowledge they need to make the best possible decisions with their patients.
